Amnesiac milkman John Doe (Anthony Mackie) transports goods across the wasteland between cities and is tasked with delivering a mysterious package from New Chicago to New San Francisco. He teams up with Quiet (Stephanie Beatriz) and as they travel together they encounter multiple threats including a psychotic killer clown.
